Together with your broker, EBMS will evaluate your group’s historical claims data and reports to make benefit plan recommendations.  These will consider your group’s unique situation and may include one of the following population-based health management programs.
 
Disease Management  (Return on Investment 3:1) People with chronic conditions account for 83% of all healthcare spending.
Managing for Tomorrow is a disease management program that applies the principles of influence and behavior modification to assist patients suffering from asthma, diabetes and cardiac disease. Candidates for the program are identified through analysis of claims history and they are invited to participate in the health management program.

Priority Maternity Care is an educational and empowerment program for eligible female employees or dependents. This program provides a means to positively affect a pregnancy and health of the baby. A nurse interacts with the pregnant woman to assist and coordinate high-risk aspects of maternity care, including information regarding access to educational programs and community resources. An informative calendar, and two books “Baby and Me” and the “First Year of Life” are sent to the participant.
 
Nurse Hotline
EBMS CARE LINE is a nurse hotline and audio library. CARE LINE provides 24/7/365 access to consult with a nurse about health concerns. Included in the service is an audio library of over 1,100 health topics ranging from wellness to croup to cancer.
