Healthcare Index — Analysis of group population healthcare consumption. Each group will receive an index based upon the need for medical services associated with their age and gender demographics and the chronic illnesses present in the population.

Predictive Modeling — Science of ranking individuals from those with the greatest probability of disease onset to the least probability. Provides important data on those individuals who are most likely to incur high dollar claims within the next 12 months in order to create an opportunity for early identification, intervention and coaching.

Health Reminders — Manage the utilization of the 20% of the employee population driving 80% of the costs. Individuals identified with one of nineteen high-risk conditions are directed into this care management program.

Provider Profiling — Measurement of physician’s practice patterns, cost efficiency and preventive care service rates. CareLink nursing staff will use this data to coach and educate employees to ensure they are utilizing cost-effective, high-quality providers of care.

WORLDDOC — WorldDoc is an interactive, web-based system that helps Plan sponsor improve the health of their population and control health costs. WorldDoc features a Health Risk Assessment, personal health evaluation system, medical library, and a 24/7 nurse hotline, along with other valuable information.

What do we do with the data?

Your dedicated EBMS Team will evaluate your group’s Healthcare Index and Predictive Modeling data with additional statistical reports, to make appropriate benefit plan recommendations to the plan sponsor. The CareLink nursing staff will coach and educate employees to become better consumers of healthcare, and place these members in the medical management program that can offer the most benefit.
